1. Hello Sleeping Beauty! How easily do “Golden Slumbers fill your eyes?’  

a. I fall asleep before my head hits the pillow.
b. My aches and pains keep me tossing and turning.
c. Sleep? Sleep time is more like ‘worry time” for me. 
d. It takes me a while to wind down, but I eventually drift off. 

2. If you don’t move it, you lose it! How are you movin’ it these days? 

a. I get my exercise walking to and from the car, straightening up the house, and going up and down the grocery aisles.
b. I set aside 30 minutes of exercise 3x/week doing something that gets my heart rate up like walking, tennis, biking or hiking. 
c. I work out for 45-60 minutes 3-4x/week at something I enjoy.
d. I am bored and uninspired by my workout routine. 

3. “It’s more than a feeling…”  How do you actually feel about exercise? 

a. I do it, and I like it.
b. I don’t like it, but I do it.
c. I don’t like it, so I don’t do it.
d. I want to do it, but I can’t find the time.  

4. Let’s do lunch! What will you most likely order?

a. A slice of pizza and a coke.
b. A cup of coffee and a muffin.
c. A chicken Caesar salad.
d. A deli sandwich.

5. Mom always said to eat your vegetables. Are you doing her proud?

a. No. I never liked vegetables, and I still don’t.
b. No. I eat some veggies but not enough.
c. I think so. I eat at least 2 servings a day.
d. Absolutely! I eat 3-4 servings a day.

6. Let’s not wait to discuss weight! How’s yours?

a. It’s ideal.
b. It hasn’t changed in years, and that must be a good thing.
c. I could lose some, that’s for sure.
d. Mine prevents me from moving forward confidently in my life. 

7. Friends know best! When it comes to handling stress, how would your friends describe you?

a. Keyed up, nervous and jittery.
b. Calm, cool and collected.
c. Keeps it together pretty well but has an occasional meltdown.

8. If we had a crystal ball, what would we find you doing in the near future?

a. Like the energizer bunny; it just keeps on going..
b. Like a puppy; bursts of energy and then dog tired
c. Like a tortoise; slow and steady wins the race.
d. Like a three-toed sloth; barely moving and good at playing dead!
